Mehak Homes welcomes referrals from local authorities, NHS services, social workers, care coordinators, commissioners, discharge teams and other healthcare professionals seeking safe and appropriate placements for adults requiring specialist support.
We recognise the importance of responsive and effective placement planning and aim to work collaboratively with professionals to ensure each referral is assessed thoroughly and efficiently. Our team carefully reviews the individual’s care needs, risks, support requirements, compatibility and long-term goals to determine whether we can provide a suitable and positive placement outcome.
Our service is designed to support individuals with a range of complex needs, including learning disabilities, autism spectrum conditions, mental health needs, emotional and behavioural difficulties and behaviours that may challenge services. We aim to provide structured, stable and person-centred support that promotes safety, wellbeing, emotional stability and increased independence.
We understand that transitions into care settings can often be difficult and stressful for individuals and families. For this reason, we place strong emphasis on careful transition planning, communication and relationship-building to help individuals settle comfortably into their new environment while maintaining continuity of care and support.
Our team is committed to maintaining open communication throughout the referral and admission process and will work closely with all relevant professionals to ensure placements are managed safely, professionally and with the individual’s best interests at the centre of all decisions.
